COVID-19, adrenal gland, glucocorticoids, and adrenal insufficiency

COVID-19 can alter adrenal gland function by multiple mechanisms. And also, it is predicted that there may be a critical illness-related change in the pharmacokinetics of glucocorticoids in COVID-19 infection. These effects should be taken into account when using glucocorticoids in COVID-19 patients with or without previously known adrenal insufficiency. In patients with adrenal insufficiency, a more dynamic approach and close follow-up are required for glucocorticoid replacement in patients with COVID-19.

Heterotopic acinic cell carcinoma and its clinical implications

Heterotopic acinic cell carcinoma originates primarily in intra-parotid, para-parotid or suprahyoid cervical lymph nodes. Because of its extremely rare occurrence, the management is largely intuitive. In the article, literature review and current comprehensive analysis of clinical and histopathologic characteristics of "orthotopic" acinic cell carcinoma were used to establish a rational treatment strategy for heterotopic acinic cell carcinoma.

Controversies of radiotherapy in human epidermal growth factor receptor (HER)-2 positive breast cancer patients

Data indicate that biological factors are important determinants in the prognosis in breast cancer. In the past, HER-2-positive breast cancer patients treated with mastectomy and radiation generally had poor outcomes, including high LRR rates. Significant improvement in radiotherapy was achieved in patients with HER2-positive tumors treated with trastuzumab. This is more pronounced in patients with advanced HER-2-positive breast cancer. Studies have demonstrated significant benefits of postmastectomy radiotherapy in patients with ER-positive HER-2 positive tumors compared to ER-negative, HER-2-positive breast cancer.

This study provides unique epidemiological and mortality data of patients discharged from acute heart failure hospitalization, and confirms their poor prognosis despite developments in treatment management. HFrEF patients have higher cardiovascular mortality than HFpEF patients but the all-cause mortality and rehospitalization rates are similar in HFrEF and HFpEF patients.

Out-of-hospital cardiac arrest is a priority emergency for emergency medical services. The success rate of cardiopulmonary resuscitation is a fundamental parameter of the quality of provided pre-hospital emergency care. The study confirmed that the epidemiology of out-of-hospital cardiac arrest in the Pilsen region corresponds to the data from other studies worldwide. The resulting survival rate with good neurological score was found to be higher than a number of other studies.

In this study, we analysed the findings in sentinel node biopsy after surgical treatment in breast cancer patients. The aim of the analysis was to determine correlation between preoperative axillary ultrasound and metastases in sentinel node biopsies and between type of primary breast cancer and metastases in sentinel node biopsies. This could contribute to accurate diagnosis of metastases in axillary lymph nodes with minimal morphological changes and could contribute to management of targeted axillary dissection.

Our finding of a simple immunohistochemical panel with antibodies against p53, oestrogen and progesterone receptors is a very useful ancillary method in the diagnosis of atypical endometriosis and also very important for clinical purposes - correct diagnosis of atypical endometriosis is followed by careful long-term observation due to possible subsequent development of neoplasia.

The influence of central corneal thickness on progression of normotensive glaucoma

The paper reviews the effect of prostaglandin treatment or no treatment in patients with normotensive glaucoma (NTG) on the progression of functional changes in the eye. No similar study could be found in the literature. Another priority of the work was the impact of CCT (central corneal thickness) on the progression of NTG. In hypertensive glaucoma, the effect of CCT on progression was demonstrated. This was not demonstrated in NTG. This conclusion also points to different diseases.

CYP2A6 and EGLN2 variants were significantly associated with plasma concentrations of 3OH-cotinine and 3OH-cotinine:cotinine ratios. Neither genetic variants nor concentrations of individual nicotine metabolites and nicotine metabolite ratios were associated with abstinence rates.

Infrared thermography distinguishes healthy patients from patients with Raynaud's phenomenon, and then capillaroscopy can be used to differentiate primary Raynaud's phenomenon from secondary Raynaud's phenomenon. Infrared thermography can detect which fingers are more affected, and these can then direct the focus of capillaroscopy

Radiotherapy dose limit for uterus fertility sparing in curative chemoradiotherapy for rectal cancer

The authors report the first case of full-term delivery of twins after curative chemoradiotherapy for anorectal adenocarcinoma T2 N0 M0 with a total dose of 58.6 Gy (after in vitro fertilisation procedure) 10 years after the radiotherapy treatment. The applied dose to the uterus allows us to propose an increased limit dose enabling fertility sparing beyond the limits achieved from total body irradiation series with 12-14 Gy and accept 16 Gy as the uterine body (35 Gy for uterine cervix) limit for IMRT treatment planning in young patients asking for maintaining fertility potential.
