Even though accurate estimation of arterial stiffness in AoS is challenging, the coexistence of an increased pulsatile load may influence patient outcome before as well as after aortic valve replacement. |
The use of sentinel skin islands for monitoring buried and semi-buried micro-vascular flaps. Part I: Summary and brief description of monitoring methodsMartin Molitor, Ondrej Mestak, Richard Pink, Ren Foltn, Andrej Sukop, Stefano LucchinaBiomed Pap Med Fac Univ Palacky Olomouc Czech Repub. 2021, 165(2):113-130 | DOI: 10.5507/bp.2021.016 Over 40 monitoring methods of micro-vascular flaps have been reported in the scientific literature and it is difficult to discern the benefits of these over other methods or over direct visual monitoring, the standard method. In this review, we collated all reported monitoring methods to date and critically assessed their value including for practical use. We found that direct visual monitoring remains the most used, is very reliable and cheapest method for micro-vascular flap assessment. Only Doppler ultrasound, laser Doppler flowmetry and near infrared spectroscopy show any evidence of greater value but they require special equipment, experienced personnel and are more expensive. |
Adherence of obstructive sleep apnoea syndrome patients to positive airway pressure therapy - 10-year follow-upSamuel Genzor, Jan Prasko, Jakub Vanek, Amjad Ghazal Asswad, Shayan Nadjarpour, Milan SovaBiomed Pap Med Fac Univ Palacky Olomouc Czech Repub. 2022, 166(4):441-446 | DOI: 10.5507/bp.2021.041 Continuous positive airway pressure (CPAP) is the most effective therapy for obstructive sleep apnoea syndrome (OSAS). The aim of this study was to assess the ten-year adherence to CPAP therapy, and to define the factors influencing it. Sufficient adherence, which was defined as more than 4 hours of CPAP use per night, was found in 57% of patients in the first year with no subsequent significant change over the next 9 years. Adherence to therapy correlated significantly with the severity of OSAS and Epworth Sleepiness Scale score but did not correlate with the pressure used or the age of the patient. |

The use of sentinel skin islands for monitoring buried and semi-buried micro-vascular flaps. Part II: Clinical applicationMartin Molitor, Ondrej Mestak, Richard Pink, Rene Foltan, Andrej Sukop, Stefano LucchinaBiomed Pap Med Fac Univ Palacky Olomouc Czech Repub. 2021, 165(2):131-138 | DOI: 10.5507/bp.2021.017 Direct visual monitoring of micro-vascular flaps is still generally and preferentially used. In cases where the flap is not exposed - buried or semi-buried flap, direct visual monitoring is not straightforward. However, apart from a few exceptions, each micro-surgical flap can be designed as a composite which includes a skin island that can serve as a direct monitor of flap viability. This review describes all the published techniques for externalising part of a buried flap. It also defines which tissues are suitable to be used as an externalised component of the flap and summarises the principles that need to be observed to achieve the greatest reliability and validity of the externalised flap's segment monitoring. |
COVID-19, adrenal gland, glucocorticoids, and adrenal insufficiencyEmin Murat Akbas, Nergis AkbasBiomed Pap Med Fac Univ Palacky Olomouc Czech Repub. 2021, 165(1):1-7 | DOI: 10.5507/bp.2021.011 COVID-19 can alter adrenal gland function by multiple mechanisms. And also, it is predicted that there may be a critical illness-related change in the pharmacokinetics of glucocorticoids in COVID-19 infection. These effects should be taken into account when using glucocorticoids in COVID-19 patients with or without previously known adrenal insufficiency. In patients with adrenal insufficiency, a more dynamic approach and close follow-up are required for glucocorticoid replacement in patients with COVID-19. |
