Comparing Endosonography-Guided Fine-Needle Aspiration (EUS-FNA) and Key-Hole Biopsy (KHB) in tissue diagnosis of upper gastrintestinal SET, the authors confirm that both deep biopsy methods are safe, high-yielding, and equally useful for deep tissue sampling of the upper gastrointestinal SET . EUS-FNA and KHB could be recommended in the future, for the purposes of confirming cytological / histological diagnosis of upper gastrointestinal SETs ≥2 cm, where pathological diagnosis is a sine qua non for management. They also conclude that neither methods can predict the prognostic mitotic index in the case of gastrointestinal stromal tumors.

Carcinoma of the oral cavity is connected with the human papillomavirus. A number of surveys have found that the origin of oropharyngeal cancers is etiologically associated with this high-risk HPV, but in OSCC the presence of HPV is uncommon. Most studies have shown high prevalence of HPV in OPCS (73%), but patients with OSCC have a low infection rate of HPV (4%). Our clinical experience is that there is a marked difference in the prevalence of HPV positivity in OPCS and OSCC patients.

Mucosal administration systems offer an alternative to the oral route when the latter is not tolerated, contraindicated or refused. Perindopril arginine and Amlodipine besylate are registered in the EU in the form of orodispersible tablets for mucosal delivery, however, they are not available in all countries. Antihypertensive drugs Captopril and Nitrendipine administered sublingually are commonly used in hypertensive urgencies. Metoprolol tartrate suppositories have only been tested in pharmacokinetic studies in humans. Propranolol suppositories have been reported clinically in a case report. Furosemide suppositories have been tested in animals, but there are no reports of their use in humans. New insights into a practical approach: a) First use of Furosemide-containing suppositories in a human subject. b) A successful regimen of four different antihypertensive drugs administered long-term via the mucosal route (rectal Furosemide and Metoprolol tartrate combined with sublingual Captopril and Nitrendipine).

Bone is the second most commonly transplanted tissue. For large-scale bone injuries, it is necessary to use optimal bone replacements, often made of titanium or titanium alloys. Insufficient healing can lead to serious complications. The surface of a titanium implant needs to be treated to increase its roughness. This will improve cell adhesion and following implant osseointegration. In this review, the authors explain and compare methods currently used for treating titanium implants aimed at improving the bone-implant contact and reducing the risk of complications.

This investigation characterizes the features of a healthy subject's peripheral blood B lymphocytes infected with Epstein-Barr virus (EBV) using the multi-parametric flow cytometry technique. EBV-infected B cells are predominantly immunoglobulin M (IgM)+ memory or IgA+ plasmablasts. In comparison to uninfected counterparts, the population of EBV-infected B cells is significantly reduced for upper respiratory tract homing α4β1 integrin and enriched for peripheral lymph node targeting CCR7 positive cells, indicating significant deviation in their migratory and maturation profile.

Retroperitoneal tumors are relatively common, the impairment spectrum is broad, and safe establishment of correct diagnosis is crucial for the planning of further therapies. CT-guided biopsy using a percutaneous approach is a suitable interventional method with comparable diagnostic accuracy to surgical excision, but significantly less invasive.

Gene rearrangements have been identified in patients with non-small cell lung carcinoma (NSCLC). Early detection of these biomarkers is crucial for targeted therapy determination. Currently available methods have limitations, that can be overcome by next-generation sequencing. This review compares current methods of predictive gene rearrangements detection in patients with NSCLC.

Total hip replacement (THR) is frequently followed by considerable pain. Epidural analgesia is considered the technique of choice in the postoperative care of patients after THR. We found that patient-controlled epidural analgesia PCEA decreases total analgesics consumption compared to conventional method analgesia based on physician's prescription while maintaining similar pain relief and safety profile. PCEA should be considered first choice method of analgesia in patients undergoing THR.

In this study, the authors derived a risk score for prediction of persistence of perfusion defects after pulmonary embolism (PE). Based on a long-term follow up of 83 patients after PE, three risk groups were identified according to the level of hemoglobin and age at the time of PE.

Although the qPCR method used was able to reliably detect at least 4 copies of the E6 gene per reaction in HPV16, 18, and 31, and 40 copies per reaction in HPV56, no hrHPV DNA was found in primary non-small cell lung cancer tumors in the Czech population.

In this review, the authors have assembled the literature on neurophysiological and functional imaging sequelae of mechanical pressure stimulation motivated by its clinical applications, including reflex locomotion therapy. The authors highlight the limited amount of research devoted to this stimulus modality, emphasise current knowledge gaps, present recent developments in the field and highlight the evidence awaiting replication or confirmation in future neuroimaging studies.

Radiation therapy is rapidly evolving and modern techniques such as deep-inspiration breath hold and stereotactic radiotherapy are often time consuming. This is the first study investigating the time factor associated with different forms of DIBH. The authors show that audio-visual navigation significantly reduces the treatment time.

The aim of this study was to compare the efficacy of the Pleuralvent™ device and classic chest tube, in which the clinical data in the treatment of iatrogenic pneumothorax were lacking. In this pilot study, the authors are the first to assess the genetic architecture of recent-onset dilated cardiomyopathy in 83 individuals from the region of Moravia in the Czech Republic by whole-exome sequencing (WES). WES detected at least one disease-related variant in 54% of patients, highlight the genetic origin of the disease. Individuals with disease-related variants of non-titin genes showed impaired left ventricular reverse remodelling, a marker of poor prognosis, at 12-month follow-up compared to individuals with no genetic cause.

This review deals with current issues of uterine transplantation (UTx) in the treatment of female sterility caused by absolute uterine factor infertility. This promising method of treatment is still at an experimental stage worldwide. Current controversial areas of this treatment are discussed in terms of the interest in UTx, the individual risks to the recipients, the use of living or deceased donors and their parity, the surgical risks to the donors, the venous outflow from the uterus, the collection of embryos before transplantation, the reproductive success, and the creation of an international registry of infertile women treated with both uterine transplantation and assisted reproductive technologies.

The IGF-IGF-1R axis plays a role in the development of papillary and anaplastic thyroid cancers. Serum concentrations of IGF-1 and IGF-1R may thus be considered additional markers for the differentiation of malignancies during the preoperative diagnosis of patients with thyroid gland tumors.

Glioblastoma pseudoprogession is dependent, by definition, on the location within the RT high dose field, however, there is lack of routine reporting of RT isodoses to radiologists during glioma follow up. Using blinded contouring and a number of different indices, we found poor awareness among routine clinical radiologists of per-protocol constructed target volumes for adjuvant glioblastoma RT, thus, automatic tools for exporting of RT target volumes would be very useful for routine clinical practice.

Neither AF inducibility or the presence or absence of its early spontaneous termination after pulmonary vein isolation under general anaesthesia in paroxysmal AF patients are useful as predictors of arrhythmia recurrence during 12 months after ablation. The clinical usefulness of rapid atrial stimulation / AF induction in paroxysmal AF patients after pulmonary veins isolation remains questionable.

A rare case of MDR3 disease caused by two novel pathogenic mutations in ABCB4 in a young male with biochemically impaired copper metabolism mimicking Wilson disease is reported. The value of hepatic copper content (1867 µg/g dry weight) on the background of ABCB4 deficiency is the highest recorded so far.

Infectious uveitis is one possible complication in immunodeficient patients. The occurrence of cytomegaloviral retinitis is life-threatening complication in patients who have had hematopoietic stem cell transplantation. Patients with herpetic retinopathies have a poor visual prognosis.

Stereotactic body radiotherapy (SBRT) requires great accuracy in order to deliver high doses of radiation to areas close to the organs at risk. SBRT of spine tumors is challenging because of the close relationship between the target tissue and the spinal cord. The present study evaluated spine motion during SBRT treatment to assess the risk of verifying patient alignment only prior to starting treatment.

Aim of this study was to highlight the possibility of using US-PEIT to treat recurrent and symptomatic thyroid cyst. In this patient cohort, the relatively small amount of alcohol (corresponding to 20% of initial volume) was sufficient to treat the cysts, with a very low incidence of complications. To our knowledge this is the third largest published patient cohort on this issue.

MRI/TRUS fusion targeted biopsy in combination with systematic biopsy had significantly higher detection rates than systematic biopsy in the rebiopsy group only. Differences in detection rates between examiners are dependent on the level of previous experience with TRUS guided biopsy.
